CPPQ – Community Pharmacy Patient Questionnaire 

All pharmacies are required to conduct an annual community pharmacy patient questionnaire, also referred to as the Patient Satisfaction Questionnaire. The questionnaire allows patients to provide valuable feedback to community pharmacies on the services they provide. 

At Chel Pharmacy, we always try to provide you with the best possible service. We are asking NHS patients to complete a short and anonymous questionnaire about the service we provide.
